I've always been curious how people get these shots from seemingly behind the acoustic shell at the concert hall. Is there a hole in it where you can stick your lens through? Also, do you use something to muffle the mirror slap so that it doesn't distract the musicians?

There are two holes in the acoustic board behind the orchestra. They provide ladders for us to climb up to the hole and shoot.
Bass Hall doesn't require blimps (sound mufflers), but they prefer you have them. The Meyerson in Dallas has a sound-proof booth for photographers. In venues where there is no shooting area (i.e. you're in the audience), a blimp is required.
